blowing a 222rem out to 223 would be the same as taking a standard 06case, and blowing it out to a gibbs case. It will take place under that much pressure. Maybe not a perfect form with positive headspacing,..but it will appear very different when it comes out of the chamber,..at least enough to see with the naked eye.

If this isn't the case,...I would say a damaged crown, gouged rifling, or damaged rifling at the leade/throat area.
